PROJECT DESCRIPTION
The project, to be known as the Jennings City Center, consists of Two New Buildings: A New City Hall, approximately 13,000 square feet, and a New Community Center, approximately 15,000 square feet. Work includes but is not limited to, site clearing and excavation, concrete foundations and flat work, structural steel, roofing, landscaping, sanitary and storm sewers, HVAC, electrical, plumbing, and communications work.

City Center Project
Jennings, Missouri
The City of Jennings Missouri is soliciting proposals from experienced and qualified firms to provide On-Call as needed Construction Materials Testing (CMT) services associated with the construction of the New City Center Project.
MBE/WBE Utilization
The City of Jennings goal for minority and women-owned businesses is thirty (30) percent of all commercially useful functions, as measured by the percentage of the dollar value of all work on the contract, to be performed by minority business enterprises (MBE) and then ten (10) percent of all commercially useful functions be performed by woman business enterprises (WBE). The percentage of M/WBE participation will be a consideration in the selection of a Successful Bidder.
Project Description:
The Jennings City Center Project consists of Two New Buildings; A New City Hall, approximately 13,000 square feet, and a New Community Center, approximately 15,000 square feet. The two buildings will be separated by a courtyard with senior exercise areas and walking trails. Work includes but is not limited to, site clearing and excavation, concrete foundations and flat work, structural steel, roofing, landscaping, sanitary and storm sewers, HVAC, electrical, plumbing, and communications work.
INSURANCE
Provide evidence of insurance for each of the checked categories:
☒ General Liability (Including operations, products, and completed operations, as applicable.) $1,000,000 - per occurrence for bodily injury, personal injury and property damage. If Commercial General Liability Insurance or other forms with a general aggregate limit is used, the general aggregate limit either must apply separately to this project or must be twice the required occurrence limit.
☒ Automobile Liability of $1,000,000 - per accident for bodily injury and property damage.
☒ Workers ‘Compensation As required by the State of Missouri
☒ Employers’ Liability $1,000,000 - each accident, $1,000,000 policy limit bodily injury by disease, $1,000,000 each employee bodily injury by disease. ☒ Professional Liability (Errors and Omissions) $1,000,000 - per occurrence To be carried at all times during the term of the Contract and for three years thereafter. The City of Jennings shall be named as additional insured.
SCOPE OF WORK
Provide all things necessary including site visits written reports of all observations and test results and
occasional participation in construction progress meetings, when necessary, for the following scope of
work in its entirety as required by the Project Manual volumes 1-3 including Division 01 General
Requirements - 014000 Quality Requirements, and in associated technical specification sections found
under the heading Quality Assurance. Scheduling of all services will be coordinated by Hankins Construction of St. Louis, Missouri.
ANTICIPATED SERVICES
The proposed Agreement(s) anticipated by this RFP comprehensively covers services that can be defined as special inspection and/or material testing services. The services may include, but are not limited to, some or all of the following services:
a. As needed major Geotechnical Inspection and material testing services.
b. Perform a laboratory material acceptance standard test, which includes a gradation test, compaction curve, R-Value test, sand equivalent test, and durability index test.
c. Perform field observation/field density compaction testing services (nuclear gauge) and material testing services as requested, and on an intermittent basis in compliance with the project plans and specifications.
d. Document test data and observations through daily and end-of-job summary reports. Daily reports shall be submitted to the City of Jennings on a daily basis relative to the provision of services. Daily reports shall describe the activities performed at the time of field services, and the locations and results of the field density tests and other material testing performed. All report writing and office supervision expenses will also be included in the hourly rate.
e. As needed other Special Inspection and material testing services. a. Perform other as-needed special inspection and material testing services such as asphalt concrete pavement inspection, steel and Portland cement concrete inspection. Certified testers/samplers and others as needed type of special inspection and material testing services.
f. Perform sampling of cast in place concrete cylindrical specimens and test for concrete compressive strength.
g. The work shall be performed by qualified personnel with valid professional licenses and certifications. Copies of licenses and certifications shall be provided to County for verification.
Pricing
The firm shall insert the price for all RFP items and all other information requested in these specifications. The price shall be full, delivered cost to The City of Jennings including all factors whatsoever.
Total versus “Per Item” Awards.
The City generally awards contracts based on a lump sum basis to the lowest responsible and responsive proposal. However, The City may choose to award on a per-item basis. Therefore, each Responder must submit pricing for each item indicated on the RFP forms. Proposals must clearly indicate which items are included in the proposal and which are not.
Taxes
No charge will be allowed for taxes from which The City of Jennings is tax-exempt. The City of Jennings is also exempt from the Federal Excise and Transportation Tax.
Withdrawal of Request for Proposal
Firms may withdraw or cancel their Request for Proposal at any time prior to the advertised RFP opening. After the opening time, no RFP shall be withdrawn or cancelled. All proposals shall be firm and valid for a period of sixty (60) calendar days.
Subcontracting. The successful bidder shall not subcontract any part of the work without written permission from the City of Jennings.

• Confirm the proposal’s scope inclusions and qualifications.
• Provide rates for items listed below and any cost associated with other relevant charges.
• List Personnel for the project and their roles/responsibilities.
• List references for similar projects completed.
• Acknowledgement of all addenda to this RFP document.
